We are happy to announce that we accept most insurance. We are currently in the process of being added to insurance panels. We may be able to work out a single case agreement with your insurance company. Please note that your initial visit may take one to two hours due to meeting with the nurse, obtaining lab work and vital signs, filling out paperwork and meeting with the doctor.
Also please arrive 20 minutes prior to your appointment time to fill out paperwork and begin intake process prior to seeing the Doctor.
For your convenience we would be happy to mail or fax the new patient intake packet to you prior to your visit (which includes health and psychiatric history questionnaire, insurance information packet and consent forms). Please make sure that all paperwork is filled out in its entirety or your initial intake visit could be prolonged due to any unanswered questions.
